"Psychopathology should make clear what we know, how we know, and what we do not know. This is the point where that bulk of bad philosophy in psychopathological studies originates. " KARL JASPERS Since the publication of KARL JASPERS' General Psychopathology in 1913 at least, it has become obvious that a psychopathologist cannot do without philosophical reflection. If he wants to say anything about disorders of perception and thought, or of the experiencing I, to name the subjects of this book, he must know those disorders and the problems related to them. The phenomena and problems in question are not at all simply empirical (which is to say that they concern the structure and function of the organs of sense or the central nervous system), but conceptual. We have already formed some notions and prejudices concerning those subjects - perception, e. g. , is considered to be "a projection from the outside into the inward", thinking "a kind of inward talk", the I refers to "reflec- ting upon me by myself', etc. Unless we think about these ideas critically, we shall fail in describing disorders of experience, thought, and action.

Als George Huntington 1872 die nach ihm benannte Chorea beschrieb, stellte er die Trias: Erblichkeit, Neigung zu Demenz und Beginn im reiferen Alter als besonders typisch heraus, in Abgrenzung zu der 200 Jahre zuvor von Thomas Sydenham beschriebenen infek tiosen Chorea im Kindesalter. Dabei schatzt er seine Beobachtul1gen an mehreren Familien auf Long Island wohl selbst nicht besonders hoch ein, wenn er seinen Bericht beschliesst: " ... not that I consider it of any great practical importance .., but merely as a medical curiosity, and as such it may have some interest. " In dieser Zeit nahm man noch das Kleinhirn als den Sitz der Erkrankung an. Fortschritte in den folgenden Jahrzehnten betreffen die Zuord nung des Krankheitsbildes zum Striatum, dessen kleine Zellen vorzei tig untergehen. Die Beteiligung der Hirnrinde wird als Grundlage der geistigen Storungen angesehen. Mit den neuen bildgebenden Verfah ren lasst sich heute besser als mit den klassischen Rontgenuntersu chungen das Ausmass und die Verteilung der Atrophie nachweisen. Ein Symposium in Freiburg im November 1986 setzte sich zum Ziel, die Fortschritte in der Diagnostik und Therapie in den letzten Jahren zu diskutieren-und vor allem die Aussagekraft der elektrophy siologischen, biochemischen, pharmakologischen und neuropsycho logischen Untersuchungen zu erortern."
